• Normal in the developing fetus in the yolk sac and liver
• Pathological - usu in only liver and spleen
• Rarely in other sites, assoc with
1. thalassemia
2. spherocytosis
3. myelofibrosis
• Mediastinum, retroperitoneum, lymph nodes, urinary bladder, prostate and thyroid
• Endometrium (not assoc haematologic condition)
• Spinal cord with compression

• Component of fatty tissues
• Within the adrenal gland
• Normal bone marrow

• Effect on iron overload unlikely
• Monitoring of serum iron and ferritin
• Not expected to affect B cell lymphoid function
• Potential complications include rupture and haemoperitoneum
• Could be multiple and assoc with enlargement of accessory spleens
• Pre-opt diagnosis depends on CT/MRI and high index of suspicion
